Insurance Information
I am happy to verify your insurance benefits to help determine your estimated deductible and/or copay. Please note that insurance benefits are determined by your insurance company, not by me. For this reason, it is very important that you also contact your insurance provider directly to fully understand your coverage, benefits, and any out-of-pocket costs. Final responsibility for payment remains with the client.
Good Faith Estimate
Section 2799B-6 of the Public Health Service Act
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
What this means for you:
If you don’t have insurance or are not using insurance, health care providers are required by law to give you an estimate of the expected charges for services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency services, including therapy.
Your provider must give you a Good Faith Estimate in writing at least 1 business day before your scheduled service. You can also request one before you schedule.
If you receive a bill that is $400 or more above your Good Faith Estimate, you can dispute the bill.
What you should do:
Keep a copy of your Good Faith Estimate in case you need it later.
For more information about your rights, or to learn how to dispute a bill, visit www.cms.gov/nosurprises or call 1-800-985-3059.
